Patients 1 and 2 had a post mortem. In neither case did it add information to what was already known.

Can you comment on why the colon might be selectively affected by the NOMI? As it was global, would you expect other segments of ischaemia? Or is this quite predictable?
Were there other drugs that might have played a part (we always look at NSAIDs), or was there a background of some thing that suggested poor colonic health such as constipation?
Thanks for the questions. There are also series of young adults with low output states (obstetric haemorrhage, trauma, sepsis) with colonic NOMI, but no clear explanation. A possible explanation might be variation in arterial supply to the colon via the marginal artery, but this is by no means proven.
Patient 3 in our series did have constipation occasionally managed with movicol. However, as 1 in 10 children have constipation I did not draw any conclusions from this.

With regard to NSAIDs,no patients in our series were on NSAIDs previously.
